Identification Checklist

Planning to get your Forklift Licence in Brisbane? Before attending your Forklift or Order Picker Course course, it’s important to have the correct identification documents ready. Our simple identification checklist helps you prepare the required 3 forms of ID. Prepare in advance to ensure a smooth and hassle-free booking process with FMS Training.

Forklift/Order Picker candidates are required to provide 3 forms of identification, one MUST be photographic.

Forklift Licence in Brisbane – Complete Identification Checklist

All forklift candidates must provide evidence of identity to Workplace Health and Safety Queensland (WHSQ). Your combined documents must show:
  • your full name
  • date of birth
  • your signature
  • photo of yourself
All documents must be ORIGINAL as supplied and issued by an Australian authority, unless otherwise indicated below.

⛔ Foreign-issued IDs are not accepted

This includes international driving licences, foreign driver licences, foreign birth certificates, national IDs, and any other ID not listed on this checklist. Only documents listed here are accepted, as required by WHSQ. No exceptions can be made.

🌐 International students — passport is your only accepted foreign document

If you are an international student, your foreign passport is the only overseas-issued document accepted. International driving licences, foreign birth certificates, and all other overseas-issued IDs are not accepted. Your remaining documents must come from the Australian-issued items listed below.

⚠️ WHSQ compliance requirement

These requirements are set by Workplace Health and Safety Queensland (WHSQ) in line with Queensland Transport and Main Roads identity standards. All documents must be original — no photocopies, screenshots, or printed copies. FMS Training cannot make exceptions.

Evidence of Identity Documents
Three (3) evidence of identity documents can be made up of either:
  • one category A and two category B
  • two category A and one category B
At least one Category A document must be photographic and show your full name. At least one document (A or B) must show your signature and date of birth.

Note: Non-Queensland digital driver licences are not acceptable; the physical licence card must be presented.

⛔ Foreign-issued driver licences are NOT accepted — including international driving licences.

Australian birth certificate — full, not an extract or commemorative certificate

If you were born in Queensland, you can apply for a copy of a Queensland birth certificate or apply in the state you were born in.

⛔ Foreign or international birth certificates are NOT accepted.

Bicentennial birth certificate — issued for births in 1988. Must be Australian-issued.

CHANGE OF NAME

To change your name on an existing HRW licence you will be required to provide three evidence of identity (EOI) documents from category A and B (at least one needs to be from category A) and a change of name document from the below list. The documents must be in colour, legible, verified and stamped by a Justice of the Peace and can be uploaded during the online replacement application process.
