Please read the following conditions carefully, these apply to all bookings including On-site Training, and Assessments.

FMS Training reserves the right to update and change the Terms of Service without any prior notice.

IMPORTANT NOTE: By booking a course with FMS Training, you are confirming that you have read, understood, and agree to these Terms and Conditions in full.

COURSE REQUIREMENTS

Licensing Courses (e.g. Forklift and Order Picker)

Candidates must:

  • Be at least 18 years of age at the time of assessment.
  • Provide a Unique Student Identifier (USI).
  • Provide three (3) forms of IDs, including one with a photo. Please refer to the ID Checklist.
  • Wear appropriate closed toe footwear (such as joggers, trainers, runners, sneakers, or steel caps). No thongs/sandals allowed.
  • Be able to understand, read and write in English (WH&S Requirement).
  • Have a good understanding of both written and verbal English to meet safety requirements and successfully complete the assessment.
  • Be able to complete a theory assessment (without assistance of a translator).
  • Be able to complete a practical driving assessment independently.
  • Complete the Enrolment Form prior to the course start date: Enrolment Form
  • Have read and understood our Cancellation, Reschedule Policy, and Terms and Conditions.
  • Have access to a computer, tablet, or other suitable device to complete pre-course study materials, and possess reasonable digital literacy skills to access and use online learning resources.
  • Be ready and willing to study and complete the pre-course practice quiz provided upon enrolment. The completed quiz must be printed and brought with you on the day of training.

Non-Licensing Courses (e.g. Working at Heights, Confined Spaces)

Candidates must:

  • Be at least 15 years of age at the time of the course start date.
  • Provide a Unique Student Identifier (USI).
  • Provide at least one form of ID that shows your full name, date of birth, your signature, and a photo.
  • Wear appropriate closed-toe footwear (such as joggers, trainers, runners, sneakers, or steel caps). No thongs or sandals allowed.
  • Be able to understand, read, and write in English.
  • Have a good understanding of both written and verbal English to meet safety requirements and successfully complete the assessment.
  • Be able to complete a theory assessment without the assistance of a translator.
  • Be able to complete all required practical assessment activities independently.
  • Complete the Enrolment Form prior to the course start date: Enrolment Form
  • Have read and understood our Cancellation, Reschedule Policy, and Terms and Conditions.
  • Be ready and willing to study and complete the theory training and the final theory test provided upon enrolment. The completed final theory test must be printed and brought with you on the day of training.

PRE-COURSE STUDY AND DIGITAL LITERACY POLICY

By booking any course with FMS Training, you acknowledge and agree to the following requirements:

  • Licensing Courses (e.g. Forklift and Order Picker):
    • You must complete all required pre-course study and preparation prior to attending training.
    • This includes watching instructional videos, reviewing training materials, and completing the pre-course practice quiz provided upon enrolment.
    • The practice quiz must be printed, answered in full, and brought with you on the day of training.
  • Non-Licensing Courses (e.g. Working at Heights, Confined Spaces):
    • FMS Training provides practical training and assessment only.
    • Before attending your course, you must complete the theory training and the final theory test emailed to you upon enrolment.
    • The final theory test must be printed, answered in full, and brought with you on the day of training.
  • All pre-course study materials and resources are delivered digitally. You confirm that you have access to a computer, tablet, or other suitable device, and you have reasonable digital literacy skills to access, read, and complete online learning content.
  • You accept responsibility for ensuring you are adequately prepared for your training and assessment. Failure to complete and bring the required paperwork will not be accepted as a reason for rescheduling, refund, or any other adjustment to your booking.
  • Candidates must provide a valid email address and mobile number and are responsible for checking their inbox and spam/junk folders for all course communications, including enrolment confirmations, pre-course study materials, and reminders.
  • These requirements are in place to comply with WHS QLD licensing standards and to ensure you are properly prepared to participate safely and successfully.

FITNESS & PHYSICAL CAPACITY POLICY

By booking a course with FMS Training, you acknowledge and agree to the following requirements regarding your physical fitness and capacity to safely participate in training and assessment:

  • You must be physically capable of completing all practical tasks relevant to the course you are enrolled in. For example, you must be able to wear a harness, climb, bend, lift, operate machinery, or work at heights where required.
  • You must disclose any existing or previous medical conditions, injuries, or health issues that may impact your ability to safely participate in training or assessment activities. This includes (but is not limited to) conditions affecting mobility, balance, vision, hearing, heart health, or respiratory function.
  • FMS Training reserves the right to request a medical clearance or further information if there are reasonable concerns about your ability to safely complete the course requirements.
  • If, on the day of training or assessment, you are deemed unfit or unsafe to participate, you will be refused entry. This will be treated as non-attendance, and all fees will be forfeited. Please refer to our ‘Non-attendance Policy’ for further details.
  • It is your responsibility to ensure you are fit for training and able to perform all required tasks without risk to yourself or others.

FATIGUE MANAGEMENT POLICY

To ensure a safe, compliant, and productive training environment, all candidates must meet the following requirements:

  • You must arrive well-rested, alert, and fit to participate fully in all training and assessment activities.
  • You must not attend training if you are fatigued to a degree that impairs concentration, reaction time, or decision-making, as this poses a significant safety risk when operating machinery, working at heights, or completing practical assessments.
  • If you are deemed unfit or unsafe to participate due to fatigue, you will be refused entry and sent home. This will be treated as non-attendance, and all fees will be forfeited. Rebooking will require payment of the full course fee.
  • It is your responsibility to ensure you are adequately rested and fit for training to avoid any disruption, refusal of entry, or additional costs. Please refer to our ‘Non-attendance Policy’ for further details.

NON-ATTENDANCE POLICY

If a candidate or group does not attend their scheduled training or assessment, this will be treated as non-attendance.

  • No refunds will be issued under any circumstances, regardless of the reason for non-attendance, including illness, personal scheduling conflicts, work commitments, or inability to attend after digital access has been provided.
  • Like an airline seat, once your booking is confirmed and the course date approaches, we are unable to resell your place. Cancellations, reschedules, or non-attendance at short notice will result in forfeited fees.
  • Purchase orders/invoices must still be paid in full, even if the candidate does not attend.
  • Candidates who arrive fatigued or physically unfit to safely participate, as detailed in our Fatigue Management Policy and Fitness & Physical Capacity Policy, will be refused entry and treated as non-attendance.
  • If you are unable to attend, you may consider nominating a substitute candidate to attend in your place. Please see our ‘Substitute Candidate Policy’ for details.
  • Failure to bring required ID or appropriate footwear will be treated as non-attendance.
  • If you attend training but do not attend your scheduled assessment, you will still be required to pay the full course fee to rebook. Missing the assessment does not reduce your rebooking cost, as your place must be reserved again for both training and assessment, occupying the same capacity as a full-paying candidate.

Upon enrolment, candidates are granted immediate access to our online theory content, instructional videos, and training resources. As this provides substantial value in the delivery of our service, all bookings are final and non-refundable under any circumstance.

If a candidate wishes to rebook after a non-attendance:

  • The full course fee must be paid again to rebook training and assessment.
  • All rescheduling requests are subject to availability once payment has been received.

LIMITED COURSE CAPACITY

To maintain a high standard of training, support our candidates effectively, and ensure a high pass rate, FMS Training strictly limits each Licensing Course (Forklift and Order Picker) to a maximum of 10 participants per session. This limited capacity means that once your booking is confirmed, your place is reserved exclusively for you.

As a result, cancellations, reschedules, or non-attendance prevent us from offering that place to another candidate and impact our ability to deliver training efficiently and maintain consistently high pass outcomes. This is why all bookings are final and non-refundable, and why full course fees apply to rebook if you do not attend as scheduled.

SUBSTITUTE CANDIDATE POLICY

As an alternative to cancellation or rescheduling, FMS Training allows for a substitute candidate (such as a colleague or friend) to attend in place of the originally booked candidate.

  • All substitutions must be arranged prior to the original course start date and confirmed in writing by FMS Training.
  • The substitute candidate must confirm they have had adequate time to review all pre-course training materials and complete any required pre-course study and paperwork before attending.
  • The substitute candidate must attend the same original training and assessment dates booked.
  • No additional fees apply for substitutions, provided the candidate meets all course entry requirements (including age, IDs, USI, language proficiency, footwear, fitness, and digital literacy).
  • Substitution requests submitted on the day of the course start date will not be approved and will be treated as non-attendance.
  • By approving a substitution, both the original candidate and the substitute confirm that the substitute candidate has been provided sufficient time to prepare and accepts responsibility for completing all pre-course requirements.

These requirements are in place to comply with WHS QLD regulations for scheduling High Risk Work Licence assessments and to ensure training places are managed fairly.

COOLING-OFF PERIOD POLICY

FMS Training does not offer or provide a cooling-off period for any bookings.

All bookings are final upon confirmation. For more information, please refer to our ‘Cancellations and Reschedules Policy.’

LATECOMER/S POLICY

Punctuality is essential to ensure the best learning experience for all participants and to comply with strict assessment conditions set by Workplace Health and Safety Queensland (WHS QLD).

FMS Training reserves the right to refuse entry to any candidate who arrives late to their scheduled training or assessment.

Candidates must arrive on time for both training and assessment sessions. Any candidate who arrives late on the day of their assessment will not be permitted to sit the exam. This will be treated as non-attendance, and all fees will be forfeited.

These requirements are in place to ensure compliance with WHS QLD regulations and to maintain the integrity and safety of the assessment process.

INSUFFICIENT IDs OR INCORRECT FOOTWEAR POLICY

All forklift candidates are required to provide three (3) forms of identification, including at least one photographic ID, and must wear appropriate closed-toe footwear (e.g. joggers, trainers, or sneakers). Thongs, sandals, or open footwear are not permitted.

Failure to present the required IDs or wear appropriate footwear on the day of training or assessment will be treated as non-attendance. Please refer to our ‘Non-attendance Policy’ for further details.

CLOSED BOOK POLICY FOR THEORY ASSESSMENTS

All theory assessments, including Knowledge and Calculations exams, are strictly conducted as closed book exams. This means candidates are prohibited from using any external materials, including notes, books, or electronic devices, during the assessment.

The use of mobile phone calculators is not permitted. Only a basic calculator, without memory storage capabilities, is allowed for use during the assessment.

By participating in the training and assessment process, candidates agree to adhere to this policy. Any violation of this rule will result in the immediate termination of the assessment, and the candidate will be required to reschedule at their own expense.

ORAL ASSESSMENT POLICY

FMS Training offers one-on-one oral assessments for candidates with Language, Literacy, and Numeracy (LLN) support needs, such as individuals with learning disabilities including dyslexia, dysgraphia, or processing disorders.

This assessment format is conducted by a licensed forklift assessor and is specifically designed for candidates who may have difficulty with reading and writing but can still demonstrate a strong command of spoken English, which is essential for safety and compliance in high-risk work environments.

  • The oral assessment fee is $350.00 (incl. GST) per person.
  • All candidates must first complete the LLN Pre-Training Test to determine eligibility.
  • This assessment option is offered by appointment only, is subject to assessor availability, and must be scheduled well in advance.
  • Places are limited, and candidates will be placed on a waiting list until a suitable session becomes available.

If a candidate is deemed Not Yet Competent (NYC) in any part of the oral assessment, including Knowledge, Calculations, or Performance, a $350.00 (incl. GST) oral assessment fee will apply to organise another assessment, which includes an additional training day, which is required under WHS QLD’s regulations for scheduling High Risk Work Licence assessments.

ASSESSMENT RESIT AND REASSESSMENT POLICY

Candidates who are deemed Not Yet Competent (NYC) in any part of their assessment (Knowledge & Calculations or Performance) will be required to complete additional training and pay the applicable reassessment fees.

  • No free assessment resits are provided.
  • All candidates are required, in accordance with WHS QLD’s High Risk Work Licensing requirements, to attend another training session prior to any reassessment.
  • The fee to organise and conduct another assessment is $350.00 (incl. GST) per person.
  • All resit and reassessment bookings are subject to availability and will only be confirmed once full payment has been received. Bookings must be scheduled by contacting FMS Training via email.

Assessment Scope

  • If reassessment occurs within two (2) calendar months of the initial assessment, candidates will only need to complete the specific components in which competency was not demonstrated (e.g. Knowledge, Calculations, and/or Performance).
  • If reassessment occurs after two (2) calendar months, the candidate will be required to complete the entire assessment (Knowledge, Calculations, and Performance).

All reassessments must be booked and completed within six (6) months of the original course date. After this period, reassessments will not be offered, and the candidate will be required to enrol in the full Forklift Course again at the full course fee to ensure compliance with the latest WHS QLD theory and practical standards.

ON-SITE FORKLIFT COURSE REASSESSMENT POLICY

If a candidate is deemed Not Yet Competent (NYC) in any part of their assessment, whether Knowledge, Calculations, or Performance, any resit will be conducted on-site at FMS Training, located at Unit 1, 42 Paisley Drive, Lawnton QLD 4501.

EARLY COMPLETION OF COURSE POLICY

Candidates who complete their course ahead of the nominated timeframe will not be eligible for any discounts or refunds. Early completion does not alter the course fee or entitlements.

ADDITIONAL TRAINING / SUPPORT POLICY

FMS Training is committed to offering flexible support for individuals with disabilities and diverse learning needs.

If the trainer determines that a candidate with a disability or diverse learning needs requires additional study or practical experience before their initial assessment, the candidate may be invited to attend an additional day of training.

  • The fee for this additional training support is $200.00 (incl. GST) per person for one full day of training.
  • This fee must be paid in full to organise another training date.
  • Additional training sessions are managed on a waiting list and are subject to availability, as another place in the training session must be booked.
  • This additional support option is only available before a candidate’s initial assessment and is offered at the discretion of the trainer.

Note: Candidates who do not achieve a competent result on their first assessment attempt will be subject to the Assessment Resit and Reassessment Policy, which requires both the mandatory extra training and the assessment resit fee ($350).

By booking the Forklift Course, you acknowledge and agree that you are willing and able to study and prepare prior to the course start date, as all instructional videos and training materials are provided upon enrolment. This allows ample time to review and be ready for your training and assessment.

HIGH RISK WORK LICENCE LODGEMENT POLICY

Upon successful completion of a licensing course (e.g. Forklift or Order Picker), candidates will be issued with a Candidate Assessment Summary (CAS). This is the official document required to apply for a High Risk Work Licence with Workplace Health and Safety Queensland (WHS QLD).

Important Notes:

  • The CAS will be sent directly to the candidate’s nominated email address shortly after successful completion of the course. It is the candidate’s responsibility to ensure their email address is correct and regularly checked (including junk/spam and trash folders).
  • It is the candidate’s responsibility to lodge their High Risk Work Licence application directly with WHS QLD.
  • Applications must be lodged within 60 days of the assessment date.
  • After 60 days, the Candidate Assessment Summary (CAS) is deemed invalid, and the candidate will be required to rebook the full Forklift Course to organise a new assessment. (Please refer to our Expired Candidate Assessment Summary (CAS) Policy for full details.)
  • FMS Training strongly recommends lodging the licence application immediately after receiving the CAS to avoid delays or additional fees.
  • The plastic High Risk Work Licence card will be posted directly to the candidate by WHS QLD.

EXPIRED CANDIDATE ASSESSMENT SUMMARY (CAS) POLICY

Candidates who fail to submit their Candidate Assessment Summary (CAS) within 60 days of their assessment date will be required to retake the full Forklift Course in accordance with Workplace Health and Safety (WHS) QLD requirements.

  • The full course fee will apply, regardless of when the reassessment is requested.
  • All candidates must attend the complete training and assessment again to ensure they are refreshed, up to date, and compliant with the latest safety standards.
  • No credit or partial transfer of previously paid fees will be applied toward the new booking.

PROMOTIONAL CODES POLICY

When booking online, please ensure you enter the promotional code and click the ‘Apply’ button before proceeding to checkout. This will ensure the special offer is applied to your booking.

FMS Training will not provide refunds if a booking is completed without activating the promo code at the time of checkout.

CREDIT POLICY

Any credit issued to a candidate’s account must be redeemed within six (6) months from the original course start date.

Credits not used within this timeframe will be deemed expired and will no longer be valid.

CANCELLATIONS AND RESCHEDULES POLICY (Effective 1 July 2025)

FMS Training maintains a strict NO REFUND POLICY for all bookings. This applies to all cancellation or reschedule requests, regardless of the reason, including illness, personal scheduling conflicts, work commitments, or inability to attend once digital access has been provided.

  • Any requests to cancel or reschedule must be submitted in writing by contacting FMS Training via email.
  • All cancellations and reschedules must be arranged at least five (5) working days prior to the course start date. (Working days do not include weekends or public holidays. Example: If your course start date is Monday, 7 July 2025, you must submit your request no later than Monday, 30 June 2025.)
  • Cancellation or reschedule requests made within five (5) working days of the course start date or on the day of the course will not be approved and will be treated in accordance with our Non-attendance Policy. (Example: If your course start date is Monday, 7 July 2025, any request submitted from Tuesday, 1 July 2025 onwards will be considered within 5 working days and will not be approved.)

These requirements are in place to comply with WHS QLD regulations for scheduling High Risk Work Licence assessments and to ensure training places are managed fairly.

By proceeding with a booking, candidates or a company representative acknowledge and agree to this policy in full. Please ensure you have booked into the correct course and dates to avoid any unnecessary fees.

CANCELLATIONS DUE TO EMERGENCY, UNFORESEEN CIRCUMSTANCES, OR ILLNESS OF A TRAINER

If a trainer or assessor becomes unexpectedly unavailable due to emergency, illness, or unforeseen circumstances (such as natural disasters), FMS Training will offer candidates the following options:

  • Reschedule: Candidates may reschedule their training to a new date that suits their availability.
  • Refund: Alternatively, candidates can cancel their booking and receive a refund.

FMS Training will notify affected candidates as promptly as possible and will do its best to minimise any disruption caused by such cancellations.

Please note: This refund or reschedule option applies only when a cancellation is initiated by FMS Training. If a candidate wishes to cancel or reschedule their booking for any other reason, the standard Cancellations and Reschedules Policy applies.

PAYMENT AND BOOKING CONFIRMATION

All fees must be paid in full prior to course commencement. Bookings are not confirmed until payment is received.

For company invoicing of Forklift Courses, the following are required before your booking can be processed:

  • A completed booking form signed by the company representative. Access the online booking form here: Booking Form.
  • An official purchase order document including the Purchase Order Number.

Once we receive these documents, we will process your booking and issue an invoice.

BOOKING ERRORS & OVERPAYMENTS POLICY

It is the responsibility of the person or organisation making the booking to ensure that all details provided at the time of booking are accurate, including the number of candidates.

  • If an overpayment is made (e.g. payment for more candidates than intended), the client must notify FMS Training in writing within 2 business days of the transaction.
  • If the additional spot(s) have not been used or allocated, FMS Training will offer to either:
    • credit the amount towards a future course booking (valid for 6 months), or
    • issue a refund, minus a $30.00 administration and processing fee.
    • No refunds or credits will be provided if the request is made after the scheduled course date, or if the additional spot has already been assigned to a candidate.
    • All requests must be submitted via email. No booking corrections will be accepted over the phone.
  • For Oral Assessment bookings:
    • Any errors in quantity or candidate names must be reported within 2 business days.
    • Once an Oral Assessment has been scheduled, rescheduling fees will apply if the slot cannot be filled.
    • Refunds or credits will not be provided for unused oral assessment bookings after the scheduled date.

FLEXIBLE REPAYMENTS POLICY — ZIP PAY (BNPL) TERMS

FMS Training offers Zip Pay as a third-party payment option for eligible bookings. From 10 June 2025, Zip Pay services are regulated under the National Consumer Credit Protection Act 2009 (Cth). By choosing Zip Pay at checkout, you agree to Zip Pay’s terms and conditions, credit assessment process, and any applicable fees.

FMS Training acknowledges the importance you attach to information that identifies you (personal identifiers such as name, address, date of birth, email address). We are committed to protecting and managing the personal information you choose to share with our organisation.

Through providing this information, we seek to ensure that you will be able to deal with our organisation in full confidence that your personal information will only be used by us in the ways we have described to you, that it will be held securely, and when there is no longer any legitimate purpose in retaining such information, it will be disposed of appropriately.

I consent to FMS Training using the information I provide and they subsequently gather, for the purpose it was collected, to assist in the administration of products and services and to carry out all necessary activities associated with their operational business activities, workplace compliance and legal governance issues. I understand that my candidate details may be viewed as part of an audit process by government officials.
